Register

We are accepting new registrations from residents living in the postcode areas listed below. To save time, you can download and fill out the registration form below and email it to us; this allows us to set up your file in advance.

Then, email the completed form to ugchuisarts@radboudumc.nl. We will process your registration immediately upon receipt. Please remember to bring a valid form of identification with you for your first visit. Our practice area is defined to ensure we can reach you promptly in case of an emergency. You may register if you live in the following postcode areas: 6512, 6521, 6523, 6524, 6525, 6531, 6532, or 6533, (not 6581)
